We encountered a case of a 49-year-old male who had both unilateral hypermature cataract and central retinal vein occlusion in his left eye. His left eye showed lens subluxation at 2 months after the first examination and central retinal vein occlusion was found after cataract surgery. Then rubeosis iridis developed rapidly in one week and resulted in neovascular glaucoma. His right eye had no abnormal ocular findings. He had no systemic abnormal finding or carotid artery stenosis. In the case of relatively younger hypermature cataract, we should consider the possibility of complications of ischemic retinopathy such as central retinal vein occlusion.
