A 48-year-old woman whose chest radiograph showed infiltration in the right lower lung field was admitted to our hospital because chest computed tomography revealed consolidation in the right lower lobe (S10). A culture of bronchial washings yielded positive results for Aspergillus fumigatus. and pulmonary aspergillosis was diagnosed. She was treated with micafungin for 3 weeks preoperatively. and subsequently underwent right lower lobectomy. During the operation, we detected abnormal arterial blood supply to the right S10; a diagnosis of intralobar pulmonary sequestration was therefore established. Pathological examination of the lung specimen revealed a cavitary lesion containing masses of A. fumigatus hyphae. but there was no finding of tissue invasion by the fungus. A patient with pulmonary sequestration associated with aspergillosis that requires surgery should receive antifungal agents preoperative.
