A case of multiple cutaneous pseudolymphoma on the head and face

A 73-year-old man presented with a two-month history of intractable itchy erythema on his face. Physical examination revealed multiple infiltrative erythema on his head and bilateral cheeks. Histopathologically, clusters of lymphocytes without nuclear atypia were scattered in the dermis, forming lymph follicle-like structures. A grenz zone was also observed under the epidermis. Immunohistochemically, most of the infiltrating lymphocytes were strongly positive for B-cell markers (CD20 and CD79a). Gene rearrangements were not detected by Southern blotting. On the basis of these findings, we made a diagnosis of multiple cutaneous pseudolymphoma. Multiple cutaneous pseudolymphoma is known to have the potential to transition into malignant lymphoma, therefore careful follow-up is required.
