A 63-year-old Japanese male presented with an 8-year history of a slow-growing painful tumor on the plantar region of his right foot. A 25x30 millimeter reddish purple nodule was seated deep in the skin. Ultrasonic and magnetic resonance imaging showed a well-defined mass in the subcutaneous tissue. Histopathological examination revealed that spindle-shaped myoid-appearing cells were present with a concentric arrangement around numerous blood vessel walls. These findings led to a diagnosis of myopericytoma. In Japanese, there is a high incidence of painful myopericytoma, according to the literature. Thus, myopericytoma should be considered in the differential diagnosis of cutaneous painful nodules.
