A 29-year-old man presented with an intractable skin ulcer expanding radially from the right leg to the ankle. He had frequent diarrhea one year previously, and was diagnosed with ulcerative colitis by colonoscopy. Based on clinical and pathological examination, the skin ulcer was diagnosed as pyoderma gangrenosum associated with ulcerative colitis in the active phase. The lesion was effectively treated with granulocytapheresis, skin grafting and systemic steroid therapy.
