A case of splenic vein thrombosis due to chronic pancreatitis

A 35-year-old male with a history of alcoholic pancreatitis was admitted to our hospital in January, 1989, because of massive melena. Upper gastrointestinal series revealed multiple submucosal tumors in the f ornix and upper body of the stomach. On endoscopic ultrasonography (EUS), these tumors were composed of round echo free spaces in the third layer of the gastric wall, which were suggestive of gastric varices. Per-cutaneous transhepatic portography showed complete obstruction of the splenic vein. He was diagnosed as splenic vein thrombosis with gastric varices, and treated by angioplasty and splenectomy. After the operation, the varices disappeared. Our case suggests that gastric varices may occur in patients with chronic pancreatitis, and that EUS is a useful procedure for the diagnosis of this condition.
