We report a case of subcutaneous nodular fat necrosis associated with pancreatic carcinoma. A 74-year-old woman noticed redness and swelling in the right leg sometime in November 2004. One week after, the same disorder appeared on the left leg. Pain then developed in the left side of the chest and back. A 4 cm diameter tumor in the pancreatic head was confirmed by abdominal CT. Skin biopsy of a representative lesion revealed panniculitis with necrotic adipocytes (ghost-like fat cells). Serum values for amylase, lipase, and phospholipase A2 were abnormally high. She was diagnosed as having subcutaneous nodular fat necrosis associated with pancreatic carcinoma. This condition is rare and is due to the release into the peripheral circulation of pancreatic enzymes because of pancreatitis or pancreatic tumor.
