脳外科術後の体位変換時に肺血栓塞栓症を発症した1例

抄録

<p>Acute pulmonary thromboembolism(PTE)is a life-threatening complication during the perioperative period. Although early detection and treatment are vital for a better prognosis, some difficulties in diagnosis exist when PTE occurs under general anesthesia. We report the case of a 57-year-old female who developed PTE after neurosurgery. Her position was changed from the lateral decubitus position to the supine position when EtCO<sub>2</sub> suddenly dropped. However, we were unable to recognize it before she developed hemodynamic instability when the transesophageal echocardiography(TEE)examination was performed. Introduction of the ECMO was planned. However, the placement of a venous cannula from the femoral vein into the right atrium induced massive emboli insertion into the right atrium and ventricle, which caused circulatory collapse. Some of these emboli were removed from the venous cannula before stabilizing the ECMO and she was successfully managed. In the process, TEE was useful not only for the diagnosis but also for prompt decision-making on treatment.</p>
