Anorexia nervosa complicated by initial irriable bowel syndrome

抄録

A 38-year-old woman was admitted to our hospital due to constipation and weight loss. She had had an outpatient treatment for somatoform disorder and irritable bowel syndrome (diarrhea type) for 4 years. A year before the admission, she complained of constipation and abdominal fullness and started a self-prescribed vegetarian diet with restricted food intake. She also engaged herself in excessive exercise. She became obsessed with food an felt that her abdominal fullness would get worse if she ate. After the evaluation of her complaints and symptoms, she was diagnosed as anorexia nervosa since she showed fear of fatness, body image distortion and amenorrhea. She was treated by cognitive behavioral therapy and showed improvement in gaining weight. This case underlines the importance of considering a disturbance of eating behavior in the management of patients with irritable bowel syndrome.
