Comparison of outcomes in partial vs. radical nephrectomy for T1 renal cell carcinoma

We performed partial nephrectomy (PNx) for T1 renal cancer. We examined operative/postoperative data, complications, prognosis and renal function. Between April 1991 and October 2007, 54 patients with renal cell cancer received PNx. The outcomes were compared with those of laparoscopic radical nephrectomy (RNx), which was performed in 57 patients from March 1999 to November 2006 in our institution. The median operating time was 225 mm. and the median estimated blood loss was 295 ml in the PNx group. The median operating time and median estimated blood loss in the RNx group were 241 minutes and 150 ml, respectively. There was no significant difference in either the operating time or estimated blood loss between PNx and RNx. The median clumping time was 32 mm. There were 2 intraoperative complications (3.7%) and 5 postoperative complications (9.2%) in the PNx group. In contrast, there were 2 intraoperative complications (3.5%) and 3 postoperative complications (5.2%) in the RNx group. The reduction rate (%) of 99mTc-Mercaptoacetyltriglycine (MAG3) clearance in the ill-side kidney were 35.5% and 33.5% at 1 week later and 3 months later in the PNx group, respectively. There were no patients with any function of the ill-side kidney. There were no patients with distant metastases or death in the PNx group.
