N2 - A 72-year-old man with a distal arch aneurysm underwent total arch replacement using an open stent-grafting technique. After the operation, a pressure difference between the upper and lower extremities developed, and emergent computed tomography revealed stenosis of the non-stented portion of the endograft. We performed an emergent endovascular aortic repair by the retrograde transfemoral approach, and the pressure difference resolved.
