Background: We performed vitrectomy in one eye of a patient with bilateral Coats disease with exudative retinal detachment. Case Report: A male who had been blind in one eye since the age of 6 years due to Coats disease presented at the age of 26 years with decreased vision in the other eye, also due to Coats disease. Ophthalmoscopic examination revealed detachment of the retina over a broad area. Although retinal photocoagulation was performed, visual acuity in this eye declined to hand motion. Vitrectomy, accompanied by internal drainage of subretinal fluid, removal of subretinal proliferative tissue, and silicone oil tamponade, was performed. Postoperative visual acuity improved to 0.07 and retinal detachment resolved. Conclusion: Vitrectomy combined with removal of subretinal proliferative tissue and internal drainage of subretinal fluid was an effective treatment in this case of Coats disease with severe retinal detachment and subretinal fibrovascular tissue.
